Hands on workshop on “Advanced Pulmonary Function Test

Lucknow desk (R L Pandey). The Department of Pulmonary Medicine at Dr. Ram Manohar Lohia Institute of Medical Sciences (RMLIMS) conducted a workshop on “Advanced Pulmonary Function Test (PFT) Workshop, on 21st September 2024.

Dr. Hemant Kumar, Head of Department Pulmonary Medicine was organizing Secretary.
The workshop covered a broad spectrum of pulmonary diagnostics, including spirometry, impulse oscillometry, FENO and body plethysmography (Body Box)
Dr. Hemant Kumar Explained about workshop contents, he told  “our department is fortunate to have these modern advanced technique Pulmonary function test machine which are available in few centres only so by this workshop we will educate other physicians of uttar Pradesh.”
He also told  “Objective & Purpose of such workshop is to avoid delay in diagnoses of important respiratory diseases”.
Workshop was started with Inaugural ceremony, chief guest was Prof. CM Singh, director, Dr RMLIMS and Guest of honour were Dean Dr Pradyumansingh, Executive registrar Dr Jyotsana Agarwal and Chief Medical superintendent Dr Ajay kumar Singh.
Dr. Ved Prakash, Head of Department, Pulmonary & Critical Care Medicine, KGMU was organizing chairman, he welcomed all dignitaries, faculties and delegates. He said “Pulmonary Department is giving excellent superspeciality services in pulmonary Medicine field specially in diagnosis of complex diseases like idiopathic pulmonary fibrosis, Emphysema, bullous lung diseases by advance pulmonary function tests etc.”
Director, Prof. C.M Singh Said -“In general practice, all patients with breathlessness are labeled as COPD or Asthma and they are prescribed inhalers or nebulizer. This practice often leads to delay in diagnosis and wrong treatment of actual disease which may be hazardous. So all pulmonary diseases specially obstructive lung disease& Restrictive Lung Disease should be diagnosed and quantified by pulmonary function test.”He also told “Dr. RML Hospital is catering large population of Eastern UP and Bihar. So patients are being referred here for better and superspeciality treatment so it is our responsibility to provide them all possible facilities. I must congratulate Dr Hemant Kumar and his entire team to provide excellent services to needy patients”

•    Dr. Hemant Kumar educated about Spirometry, He told “Spirometry is basic tool for diagnosis of obstructive and restrictive lung diseases like COPD, Asthma, Lung Fibrosis etc. it is as basic investigation as ECG for cardiologist. Though it is old device but not as popular as ECG. Reason being it required patient’s effort so difficult to do in children and elderly”.
•    Dr. Manish Verma, Professor, Physiology, Dr RMLIMS explained about normal pulmonary physiology. He told “Normal Lungs have total lung capacity of around 6 Litre which is decreases in case of ILD and muscles weakness. We take 12-15 breath/min and in each breath we take around 500 ml breath.”
•    Dr. Surya Kant, Head, Respiratory Medicine, KGMU briefly explained about pulmonary functions test. He told about history of spirometry and other PFT equipments. He told “Spirometry was discovered by John Hutchinson in 1846. In past spirometry was very bulky having big drums but now day spirometers are very handy like mobile”
•    Dr. Rajiv Garg, Professor Respiratory Medicine, KGMU explained about IMPULSE OSCILLOMETRY (IOS): He told “IMPULSE OSCILLOMETRY (IOS) is not only measure airway obstruction but also differentiate between small airway obstruction and large airway obstruction which help to diagnose Asthma/COPD even prior to development of clinical symptoms. Another benefit of IOS is that it doesn’t require patient’s effort so it is very useful in children & elderly patients.”
•    Dr. Mohammad Arif, Assistant Professor, Pulmonary & Critical Care Medicine taught about Diffusion study. He told “Diffusion study uses mixtures of gas including Carbon mono oxide gas to measure oxygen and CO2 diffusion at the level of alveolar-capillary membrane. By this method we can diagnose and measure severity of various disease like Interstitial lung disease, emphysema, Pulmonary Hypertension etc”.
•    Dr. Subodh Pandey, Professor & Head, Pulmonary Medicine, AIIMS Gorakhpur explained about Body plethysmography. He told “this machine measures certain lung volumes which can’t be measured even by spirometry like Residual volume, Functional Residual Capacity, Total Lung capacity etc.It not only helps in diagnosis of pulmonary diseases like ILD, emphysema but also help surgeons to assess amount of residual lung after Lung surgery like tumor resection, pneumonectomy, lobectomy etc.”
•    Dr. Ajay Kumar Verma, Professor, Respiratory Medicine, KGMU told about FeNO guided Asthma treatment. He told “our respiratory tract like nose, bronchi is Nitric oxide gas but very low in amount. But in cases of Eosinophilic asthma, this gas increases significantly and treatment of asthma should decrese nitric oxide gas. So eosinophilic asthma treatment should be done under guidance of FeNO level.”
Dr.Mritunjaya Singh& Dr Anurag Tripathifrom Pulmonary & Critical Care Medicine Department demonstrate various PFT machine during hands on training.
Other renowned specialists like Dr Dipti Agarwal, Dr. KB Gupta, Dr Vikram Singh, Dr Shivam Verma, shared insights on recent updates and innovations in the field of pulmonary medicine, ensuring a comprehensive learning experience for postgraduate students, faculty members, and medical professionals from across uttarpradesh.
Workshop was ended by vote of thanks given by Dr Aneeket Rastogi and Dr Sagar Jain, Senior Residents and Organizing Members, Dept of Pulmonary Medicine.
