THREE LIVES WITH MULTIPLE RISK FACTORS SAVED BY SINGLE CARDIAC PROCEDURE

Lucknow desk (R L Pandey).Pregnancy in a patient of heart disease is risky and may lead to maternal death along with fetal loss. Infact in certain heart conditions pregnancy is contraindicated
A 28 year old female with 6 month twin pregnancy from Barabanki presented to the obstetric clinic with shortness of breath. She was referred to cardiology department where she was found to have severe narrowing of one of the heart valve (mitral stenosis).
Pregnancy causes an extra burden on heart and is poorly tolerated in women with mitral stenosis and may even lead to both materal and fetal loss.
This patient was an extremely high risk pregnancy as apart from severe mital stenosis she had other high risk factors including twin pregnancy, being underweight,-weighing only 35 kg. On the top of that she was anaemic and had hepatitis C infection which carries a risk of transmission to the operators.
In a nutshell it was very critical condition requiring balloon mitral valvotomy if all the three lives were to be saved.
The patient underwent balloon mitral valvotomy procedure at Cardiology department of King George’s Medical University. Patient was operated by Dr Pravesh Vishwakarma under the guidance of Dr Rishi Sethi.
The entire team involved with the procedure included Dr Monika Bhandari, Dr Prachi Sharma, Dr Gaurav Chaudhary, Dr Akhil Sharma and Dr Umesh Tripathi. The obstetric care of the patient was done by Dr Amita Pandey, Dr Anjoo Agarwal, Dr Shalini and Dr Namrata of Obstetrics and Gynecology department.
After the procedure the patient was relieved of her symptoms and both the mother and fetuses are fine. Through this one life saving high risk procedure we were able to save three lives.
On the successful management of the patient the Vice Chancellor of KGMU, Prof Soniya Nityanand congratulated the entire team.
Prof Rishi Sethi told that operating a 35 kg pregnant women carrying twin fetuses during pregnancy is very challenging but the results are rewarding. He further informed that the patient was very poor and did not have money, so the procedure was done under Vipanna Scheme which is funded by the State government.
